EUROQoL (EQ-5D)

This Quality of Life Assessment Tool is relatively simple to administer and has been used in post-ICU populations. Although limited in scope, EUROQoL is useful tool and is available free of charge for those who register online at euroqol.

Short Form-36 (SF-36)

This Quality of Life assessment tool comprehensively reviews 8 domains of physical and mental health. It has been validated in many country’s general populations and has been used extensively in research with patients following critical illness. It can be completed as an interview or in writing. Completing the SF-36 can take up to 45 minutes. The Assessment tool is available online although registration can take time.

Nottingham Health Profile

This Quality of Life Tool was developed as a generic indicator of perceived health and has been used in various populations including ICU survivors.
